3304-5-10 Rehabilitation teacher responsibilities in the provision of IL services.

(A) The rehabilitation teacher shall provide training to consumers in communication skills, daily living skills, and in the management of secondary disabilities.

(B) Rehabilitation teacher (or a supervisor serving as a rehabilitation teacher) responsibilities in the provision of IL services shall be as follows:

(1) To carry out all requirements contained in rule 3304-5-01 of the Administrative Code that are not assigned to another party.

(2) To enter information about the consumer into a case record folder, including, to the extent pertinent, at least the following:

(a) The consumer’s request for service, the signed “Application for Services/General Consent” form with the date completed to indicate when the consumer was given or sent a copy of the form, and documentation that the rehabilitation teacher has informed the consumer of his or her rights;

(b) Documentation that the consumer has a severe visual impairment. Total blindness shall be determined through the observations of the rehabilitation teacher or the consumer stating their level of vision. When the rehabilitation teacher determines the presence of total blindness, or the consumer has so stated, the case record should include the rehabilitation teacher’s narrative report. Legal blindness or visual impairment or physical condition that will progress toward legal blindness shall be determined by medical exam or the use of a Snellen chart.

(c) The ILP.

(3) To schedule and conduct in-home interviews for all appropriate referrals and to complete an assessment regarding consumer needs and goals for independence;

(4) To provide low vision and other assessments, to coordinate all arrangements for the community-based low vision evaluation, including dates, sites, transportation, etc., and to provide counseling and advocacy;

(5) To orally discuss the ILP with the consumer and to mutually agree to the ILP and to document the discussion and agreement in the case record;

(6) To identify during initial interviews those consumers with little or no support networks;

(7) To take aggressive action to address any consumer complaints and to document the action and their results in the case record;

(8) To coordinate and facilitate group support sessions, establish goals, and provide group counseling services for consumers.
