Chapter 3364-60 Emergency Operation Plan

3364-60-04 Institutional emergency management.

(A) Policy statement

The university of Toledo shall have an emergency operations plan. The university shall make every safe and reasonable effort to provide continuous service during emergency conditions (natural, or other disasters, or loss of certain utilities, for example) or mass casualty event.

(B) Purpose of policy

To meet the mandates set forth by local, state and federal governments and relevant regulatory agencies including but not limited to the joint commission, environmental protection agency "EPA", centers for disease control and prevention "CDC", occupational safety and health administration "OSHA", national fire protection association "NFPA", department of transportation "DOT", federal emergency management agency "FEMA" and department of homeland security "DHS".

(C) Development of a plan

The institution in conjunction with the community shall develop a comprehensive emergency operations plan and procedures to deal with internal and external emergencies effecting the operation of the university. The plan shall address activities of mitigation, preparedness, response and recovery and have an all hazards approach based on the national incident management system "NIMS".

(D) Mitigation

Steps will be taken by the university of Toledo to determine the most likely emergency scenarios that could effect institutional day to day operations and provision of services in efforts to lessen their severity if they should occur.

(E) Preparedness

Content experts on campus shall come together to determine those means by which the university of Toledo can put into place procedures and individual plans based on the initial risk assessment to pre-plan and pre-stage equipment related to effective response/drills/training and education including but not limited to the following scenarios.

(1) Large scale disaster internal or external to the university

(2) Severe weather situations

(3) Large scale utility outages

(4) Bomb and other threats of weapons violence

(5) Evacuation situations

(6) Chemical, radioactive, or biological contamination incidents

(7) Fire situations

(8) Threats to individuals or groups including missing persons

(9) Terrorism events

(10) Medical emergencies including infectious disease outbreaks

(F) Response

The initiation and notification of a response to an emergency situation shall be in accordance with the designed plans and procedures. Any response efforts will be guided by the "NIMS" and an incident commander will lead the response. Response efforts should address the following issues:

(1) Rescheduling, modifying, temporarily discontinuing, or cancelling services or events.

(2) Initiate business continuity efforts to reestablish provision of vital university services.

(3) Staff support activities (i.e. housing, transportation, incident stress debriefing)

(4) Family support activities

(5) Logistics related to critical supplies

(6) Security including crowd and traffic control

(7) Communication both internally and externally

(8) Possible establishment of alternate sites to provide services and transportation of staff and supplies to site.

(G) Recovery

The plan or procedure should allow for the re-establishment of normal operations after the emergency situation has concluded. Furthermore, procedures for a proper evaluation and critique of the response to the emergency shall also be conducted to allow for implementation of constructive changes to the plan.

(H) Training

In compliance with "FEMA" guidelines, the director of environmental health and radiation safety, or designee, will determine university of Toledo employee training requirements for emergency preparedness and management. The level of training is determined by the position of responsibility and likely role the employee may have during a disaster or campus state of emergency.

3364-60-05 Hazardous material and environmental management.

(A) Policy statement

The university will manage the different types of hazardous materials (including solids, liquids and gases) in accordance with federal, state, and local regulations and other accrediting or licensing agencies.

(B) Purpose of policy

To create a campus free from uncontrolled hazardous material exposures of students, patients, staff, faculty, visitors, and in the environment surrounding the university (including air and water resources).

(C) Design and responsibility

Due to the varied nature and large quantities of hazardous materials (including wastes) a number of institutional and academic departments are be responsible for their management and control.

(1) The environmental health and radiation safety department is responsible for the overall management of hazardous materials and environmental regulatory compliance for the protection of faculty, staff, students, volunteers, patients and visitors at the university of Toledo.

(2) Environmental health and radiation safety assisted by various university departments is tasked with the development programs and plans to serve as procedural guidelines for institutional and academic departments.

(D) Methods for maintaining safety, health and environmental compliance.

(1) Proper selection, handling, storing, transporting, using, and disposing of hazardous materials and wastes from receipt or generation through use or final disposal.

(2) Providing adequate and appropriate space and equipment for safely handling and storing of hazardous materials and waste.

(3) Monitoring and disposing of hazardous gases and vapors.

(4) Identifying and implementing emergency procedures that include the specific precautions, procedures, and protective equipment used during hazardous materials incidents or exposures.

(5) Maintains documentation, including required permits, licenses, registrations and adherence to other regulations.

(6) Maintain required manifests for handling hazardous materials and waste.

(7) Properly labels hazardous materials and wastes.

(8) Maintain audit and quality control programs for the management of hazardous wastes to ensure the continued effectiveness of the management plans and programs.

(9) Develop and implement orientation and education programs for personnel who manage or have contact with hazardous materials and waste that addresses:

(a) Precautions for selecting, handling, storing, using, and disposing of hazardous materials and wastes

(b) Emergency procedures for hazardous material and waste spills or exposures

(c) Health hazards of mishandling hazardous materials
