(A) Applicants who are physical therapists, occupational therapists, speech language pathologists and audiologists, podiatrists, hearing aid dealers and fitters, optometrists, psychologists, pharmacists, pedorthists, advanced practice nurses, pharmacies, ambulances or dietitians shall be licensed, registered, and/or certified as follows:
(1) Advanced practice nurses in accordance with Chapter 4723. of the Revised Code;
(2) Ambulances in accordance with Chapter 4766. of the Revised Code;
(3) Dietitians in accordance with Chapter 4759. of the Revised Code;
(4) Hearing aid dealers and fitters in accordance with Chapter 4747. of the Revised Code;
(5) Occupational therapists in accordance with Chapter 4755. of the Revised Code;
(6) Optometrists in accordance with Chapter 4725. of the Revised Code;
(7) Pedorthists in accordance with Chapter 4779. of the Revised Code;
(8) Pharmacies in accordance with Chapter 4729. of the Revised Code;
(9) Pharmacists in accordance with Chapter 4729. of the Revised Code;
(10) Physical therapists in accordance with Chapter 4755. of the Revised Code;
(11) Podiatrists in accordance with Chapter 4731. of the Revised Code;
(12) Psychologists in accordance with Chapter 4732. of the Revised Code; and
(13) Speech language pathologists and audiologists in accordance with Chapter 4753. of the Revised Code.
(B) In addition to the requirements of paragraph (A) of this rule, services provided by eligible providers shall be requested by the child’s managing physician and shall meet the following requirements:
(1) Advanced practice nurse shall provide services as permitted under section 4723.43 of the Revised Code;
(2) Dietetic services shall be prescribed by a physician approved under rule 3701-43-03 of the Administrative Code;
(3) Hearing aid dealers and fitter services shall be prescribed by an otolaryngologist approved under rule 3701-43-03 of the Administrative Code;
(4) Pedorthist services shall be prescribed by a Physician approved under rule 3701-43-03 of the Administrative Code.
(5) Pharmacist services shall be prescribed by a physician approved under rule 3701-43-03 of the Administrative Code;
(6) Physical therapy, occupational therapy, and speech language pathology and audiology services shall be prescribed by a physician approved under rule 3701-43-03 of the Administrative Code;
(7) Podiatric services shall be prescribed by a physician approved under rule 3701-43-03 of the Administrative Code;
(8) Psychological services shall be prescribed by a physician approved under rule 3701-43-03 of the Administrative Code; and
(9) Optometry services shall be prescribed by an ophthalmologist approved under rule 3701-43-03 of the Administrative Code.
