(A) Identification numbers. Used oil transporters who have not previously notified Ohio EPA or U.S. EPA of regulated waste activity must comply with these requirements and obtain a U.S. EPA identification number.
(B) Mechanics of notification. A used oil transporter who has not received a U.S. EPA identification number may obtain one by notifying the director of their used oil activity by submitting either:
(1) A completed Ohio EPA form EPA9029 (to obtain this form, call Ohio EPA's division of hazardous waste management at 614-644-2917); or
(2) A letter requesting a U.S. EPA identification number. Call Ohio EPA's division of hazardous waste management at 614-644-2917 to determine where to send a letter requesting a U.S. EPA identification number. The letter should include the following information:
(a) Transporter company name;
(b) Mailing address of the transporter;
(c) Name of the owner of the transporter company;
(d) Name and telephone number of the transporter point of contact;
(e) Type of transport activity (i.e., transport only, transport and transfer facility, transfer facility only);
(f) Location of all transfer facilities at which used oil is stored; and
(g) Name and telephone number for a contact at each transfer facility.
