(A) An appropriate certificate shall be issued to the applicant upon completion of the qualification requirements of rule 4101:2-87-03 of the Administrative Code and payment of an initial fee of one hundred dollars. The applicant shall submit a separate fee for each certification requested.
(B) The term of the certificate shall be one year from date of approval.
(C) It shall be the responsibility of the certificate holder to renew certification.
(D) Applications for renewal of certificate shall be on forms prescribed by the board and shall be submitted at least thirty days prior to expiration of current certificate. The applicant using the “NICET” program shall show evidence of holding an active “NICET” certification in the appropriate subfield for renewal. Forms shall, in addition to other data, require the current business address and telephone number.
(E) Application forms for renewal shall be accompanied by an annual renewal fee of one hundred dollars. The applicant shall submit a separate fee for each renewal certification requested.
(F) Any holder of a certificate which has expired through failure to renew may obtain a renewal of the certificate within one year from the date of its expiration provided renewal application is made and accompanied by the renewal fee as required in paragraph (E) of this rule. The holder of a certificate which has expired shall not perform any duties for which a certificate is required.
