4112-7-01 Records and reports.

(A) Affirmative action reports and filing requirements. On or before the first day of November of each year, every employer or person subject to division (A)(10) of section 4112.04 of the Revised Code shall file an annual progress report with the commission on commission forms in conformity with the directions set forth in the form and accompanying instructions. Copies of the forms will be supplied to every entity known to the commission to be subject to the reporting requirements, but it is the responsibility of all such entities to obtain the necessary forms prior to the filing date from the affirmative action unit of the commission at "1111 East Broad Street, Suite 301, Columbus, Ohio 43205-1379."

(B) Entities required to file. Entities must file progress reports if they have undertaken affirmative action programs pursuant to:

(1) A conciliation with the commission,

(2) An executive order of the governor,

(3) Any federal statute or rule, or

(4) An executive order of the president of the United States.

(C) Penalty. Failure to file a progress report or the filing of a willfully false entry or statement in a progress report is a violation of section 4112.11 of the Revised Code and the violator is subject to a civil action for damages, injunctive or other appropriate relief, pursuant to section 4112.99 of the Revised Code.
