Rule 4723-23-09 | Information required when attesting to the competence of an applicant for a dialysis technician intern certificate or a certificate to practice as dialysis technician.

 

(A) The employer of an applicant, for a dialysis technician certificate , or the employer's designee, shall provide written information directly to the board when attesting to the competence to perform dialysis care of the applicant.

(B) The information required by this rule shall include a statement that the employer, prior employer, or the employer or prior employer's designee, in good faith affirms that the applicant has the knowledge, skills, and ability to provide dialysis care in accordance with acceptable and prevailing standards of safe dialysis care as set forth in section 4723.72 of the Revised Code and this chapter.

(C) The employer or the employer's designee shall provide assurance that the verification or statement required by paragraph (B) of this rule has been sent directly to the board by the employer.
