4723-23-11 Procedures for providing information to the dialysis technician registry.

When information is provided to the board for purposes of the registry set forth in paragraphs (A)(6) and (A)(7) of rule 4723-1-03 of the Administrative Code, the information shall be submitted to the board in the format required by the board.
