(A) A certificate shall be made available to each licensed nurse stating the licensee's name, license number, and date of issuance.

(B) The numbering of licenses shall be sequential.

(C) License certificates or other documents issued by the board as evidence of licensure, certification, or other authorization shall not be falsified or altered.
