4723-7-08 Replacement of documents issued by the board.

A licensee or certificate holder who requests a replacement copy of a card verifying completion of an approved course in intravenous therapy, or any other document issued by the board to a licensee or certificate holder that has been lost, stolen, or destroyed shall submit a notarized statement on a form specified by the board, giving proper identification, explanation of the loss, and payment of the fee as required by division (A)(8) of section 4723.08 of the Revised Code.
