4741-2-02 Notification requirement of the veterinary business facility.

The board shall be notified within thirty days after a change in the:

(A) Ownership of the veterinary business facility;

(B) Name of the facility;

(C) Address or location(s) of the facility; or

(D) Licensed veterinarian who is responsible for the management of veterinary services at the facility.
