4755-47-03 Fee for mailing lists.

(A) The board will maintain a list of currently licensed athletic trainers along with the licensees' home addresses. The fee for the list of athletic trainers license in Ohio shall not exceed the actual cost of duplication and mailing.

(B) Any person may obtain a copy of the athletic trainers license list by writing the board at: "Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board, 77 South High Street, Sixteenth Floor, Columbus, Ohio, 43215-6108," and submitting a cashier's check, business check, or money order made payable to "Treasurer, State of Ohio," in the appropriate amount.
