4761:1-7-03 Lapsed license or certificate of registration reinstatement procedure.

If a licensee or certificate of registration holder fails to renew their license or certificate of registration, the license or certificate of registration will lapse upon the expiration date. An individual may not provide HME services as defined by Chapter 4752. of the Revised Code and Chapter 4761:1-9 of the Administrative Code on a lapsed license or lapsed certificate of registration. In order for an individual to have a license or certificate of registration reinstated after the date upon which it has lapsed, the applicant must do the following:

(A) Complete an application for renewal of a license or certificate of registration;

(B) Pay the renewal fee;

(C) Pay the late fee as defined by rule 4761:1-8-01 of the Administrative Code;

(D) Comply with the renewal provisions outlined in rule 4761:1-7-02 of the Administrative Code.
