4761:1-9-04 Client records.

Licensees must maintain:

(A) Records for each client that has been sold or rented equipment;

(B) Client records must contain a physician order, if required, documentation of settings and other data relevant to the equipment that has been sold or leased to them and/or other documentation regarding service checks to the equipment sold or rented to the client;

(C) Client records must be maintained for seven years from the date of sale or in the case of a minor client, the record must be maintained for seven years after the client turns the age of majority (eighteen years of age).
