(A) "Application for Certificate of Approval Packet" or form "EMS 1650" (April, 2010)" is made available to applicants who want to operate EMS continuing education training programs pursuant to section 4765.17 of the Revised Code and Chapter 4765-7 of the Administrative Code.

(B) "Assistant EMS Instructor Initial Application" or form "EMS 5902" (March, 2014) is the application for a certificate to teach as an EMS assistant instructor, issued by the board that is made available to the program director and students. (K) "Emergency Medical Responder Refresher Training Program" (May, 2012) is a fifteen-hour course which meets all the continuing education requirements, established by the board, for renewal of an emergency medical responder certificate to practice.

(L) "EMS Instructor Initial Application" or form "EMS 0086" (March, 2014) is the application for a certificate to teach as an EMS instructor, issued by the board that is made available to the program director and students. (M) "EMS Instructor Reciprocity Application" or form "EMS 1647" (March, 2014) is the application for a certificate to teach as an EMS instructor through reciprocity, issued by the board that is made available to the program director and students. (P) "Exemption Request for Continuing Education" or form "EMS 5933" (December, 2013) is made available for certificate holders who wish to request that the board waive part of or all of the continuing education or instructional requirements needed to renew. (Q) "First Responder Training Program Ohio Approved Curriculum" (February, 2009) is a forty-eight hour course which meets the education objectives, established by the board, for initial certification as an emergency medical responder and is available at http://www.ems.ohio.gov/ems_education.stm.

(R) "National EMS Education Standards" (August, 2009) are approved by the National Highway Transportation Safety Administration and establishes the minimum performance standards, or professional competency required for service as an emergency medical responder, emergency medical technician, advanced emergency medical technician or paramedic and is available at http://www.ems.ohio.gov/ems_education.stm.

(S) "National Standard Curriculum" (EMR 1998, EMT-B 1996, EMT-I/85 1985, EMT-I/99 1999, EMT-P 2000) establishes the minimum requirements in terms of cognitive and psychomotor performance objectives related to professional competence required for services as an EMS provider as set forth for each level of responsibility, first responder, emergency medical technician-basic, emergency medical technician-intermediate and emergency medical technician-paramedic. The "National Standard Curriculum" is available at http://www.ems.gov/education standards.htm.

(T) "Ohio Approved EMS Curriculum Standards" (February, 2012) establishes the scope and sequence of an emergency medical responder, emergency medical technician, advanced emergency medical technician or paramedic training program and is available at http://www.ems.ohio.gov/ems_education.stm.

(U) "Ohio Fire and EMS Instructor Curriculum" (November, 2013) sets forth the curriculum, approved by the Board, required to be completed in order to be issued a certificate to teach and may be accessed via the division website at http://www.ems.ohio.gov/ems_education.stm.

(V) "Ohio First Responder Refresher Curriculum" (January, 2004) is a fifteen-hour course which meets all the continuing education requirements, established by the board, for renewal of an emergency medical responder certificate to practice.

(W) Physician EMS Instructor or Physician Continuing Education Instructor Initial Application" or form "EMS 0052" (March, 2014) is the application for a certificate to teach as a physician EMS instructor, issued by the board that is made available to the program director and physicians. This application may be accessed via the division website at http://www.ems.ohio.gov/ems_education.stm.

(X) "Physician EMS Instructor or Physician Continuing Education Instructor Renewal Application" or form "EMS 1648" (March, 2014) is made available to the holder of a certificate to teach as an EMS instructor who wishes to renew the certificate. (Y) "Regional Physicians Advisory Board Membership Application" or form "EMS 0096" (October, 2009) is made available to a physician who wishes to be appointed by the board pursuant to section 4765.05 of the Revised Code to oversee the delivery of prehospital EMS in a region of the state.

(Z) "Request for Extension of EMS Certificates" or form "EMS 0084" (December, 2013) is made available for certificate holders who wish to request that the board waive part of or all of the continuing education or instructional requirements needed to renew.
