4765-11-06 Notification to executive director.

Each chartered program shall provide written notice to the executive director no later than ten days after a change in any of the following:

(A) Mailing address and physical facilities;

(B) Program director name and/or contact information;

(C) Authorizing official name and/or contact information;

(D) Email address.
