(A) A fire charter or classification that has expired shall be reinstated by the executive director, if all of the following apply:
(1) A completed "Fire Charter Application" is submitted to the division.
(2) This application is submitted no later than three years after the charter expired;
(4) At the time the charter expired, the charter was not under investigation or disciplinary action for any violations of this chapter and was in good standing.
(B) An applicant whose charter expired within the time period for reinstatement is not eligible for a new certificate under 4765-11-02 of the Administrative Code.
