The department received approval effective July 1, 1990 to provide home and community-based services (HCBS) to persons aged sixty and over who require intermediate or skilled care in a program known as preadmission screening system providing options and resources today (PASSPORT).
(A) The PASSPORT HCBS waiver services and program eligibility criteria are set forth in Chapter 5101:3-31 of the Administrative Code.
(B) The maximum allowable reimbursement rates for PASSPORT HCBS waiver program services are listed in appendix A to this rule and are effective on July 1, 2011.
(C) PASSPORT HCBS reimbursement must be provided in accordance with paragraphs (A) to (C) of rule 5101:3-1-60 of the Administrative Code.
(D) A consumer may not receive community transition services with a cumulative or singular value in excess of one thousand four hundred fifty-five dollars. The consumer may only access the goods and services available through the community transition service as set forth defined in rule 173-39-02.17 of the Administrative Code.
