5101:6-20-04 Notification at application.

(A) The local agency shall inform the assistance group in writing of the disqualification penalties for intentional program violation each time the assistance group files an application for program benefits.

(B) Disqualification penalties shall be in clear, prominent, and bold-face lettering on the JFS 07200s (rev. 10/2006), sprinted copy of the information (PIC), and JFS 07501s (rev. 5/2005), which are the requisite application forms.
