A practicing license, managing license, or instructor license that has not been renewed for any reason other than because it has been revoked, suspended, or classified inactive, or because the license holder has been given a waiver or extension under section 4713.60 of the Revised Code, is expired. An expired license may be restored if the person who held the license meets all of the following applicable conditions:
(A) Pays the restoration fee;
(B) Pays all lapsed renewal fees;
(C) Submits proof satisfactory to the state board of cosmetology that the person has completed all applicable continuing education requirements;
(D) In the case of a practicing license or managing license that has been expired for more than two years, retakes and passes an examination conducted under section 4713.24 of the Revised Code for the branch of cosmetology that the person seeks to practice or type of salon the person seeks to manage.
