An existing facility issued a license under section 4717.06 of the Revised Code may transfer its existing name previously approved by the board to a new facility after complying with the following:
(A) The applicant submits a new application on a form prescribed by the board and include the appropriate license fee required under section 4717.07 of the Revised Code;
(B) The applicant demonstrates compliance with rule 4717-7-02 , 4717-7-04 or 4717-05 of the Administrative Code; and
(C) Receives approval by the board.
