Rule 5160-1-32.1 | Standard authorization form.
In accordance with section 3798.10 of the Revised Code, a standardized authorization form that meets all requirements specified in 45 C.F.R. 164.508 and, where applicable, 42 C.F.R. Part 2 as in effect on October 1, 2025, for the use and disclosure of protected health information are found in the appendix of this rule.
