Rule 4713-19-01 | Application process and fee requirements.
All applicants for a permit to operate a tanning facility shall apply on forms supplied by the board, and pay the applicable, nonrefundable fee. The fees for issuance of a tanning permit are:
(A) Seventy-five dollars for an original permit prior to June 30, 2025. Eighty-five dollars for an original permit after June 30, 2025.
(B) Seventy dollars for renewal of permit prior to June 30. 2025. Seventy-five dollars for renewal of a permit after June 30, 2025.
