Rule 4717-15-03 | Board approved certification process for crematory operation certification programs.
(A) A board approved crematory operation certification program required in division (A)(3) of section 4717.051 of the Revised Code means the following:
A certification program submitted to the board for review and approval that is approved by vote of the board.
(B) The board, after reviewing the crematory operation certification program procedures and materials, may approve the program or return it to the provider for revision. If a certification program is returned for revision, the board will indicate the reasons for the denial and the crematory operation certification program may not be offered by the provider until approved by the board.
(C) A certificate of completion issued by a board approved crematory operation certification program provider is valid for a five-year period.
