Rule 3745-205-65 | Reporting - equipment leaks.

 

(A) A semiannual report shall be submitted by owners and operators subject to the requirements of rules 3745-205-50 to 3745-205-65 of the Administrative Code to the director by dates specified by the director. The report shall include the following information:

(1) The U.S. EPA identification number, name, and address of the facility.

(2) For each month during the semiannual reporting period:

(a) The equipment identification number of each valve for which a leak was not repaired as required in paragraph (D) of rule 3745-205-57 of the Administrative Code.

(b) The equipment identification number of each pump for which a leak was not repaired as required in paragraphs (C) and (D)(6) of rule 3745-205-52 of the Administrative Code.

(c) The equipment identification number of each compressor for which a leak was not repaired as required in paragraph (G) of rule 3745-205-53 of the Administrative Code.

(3) Dates of hazardous waste management unit shutdowns that occurred within the semiannual reporting period.

(4) For each month during the semiannual reporting period, dates when the control device installed as required by rule 3745-205-52, 3745-205-53, 3745-205-54, or 3745-205-55 of the Administrative Code exceeded or operated outside of the design specifications as described in paragraph (E) of rule 3745-205-64 of the Administrative Code and as indicated by the control device monitoring required by rule 3745-205-60 of the Administrative Code and was not corrected within twenty-four hours, the duration and cause of each exceedance, and any corrective measures taken.

(B) If, during the semiannual reporting period, leaks from valves, pumps, and compressors are repaired as required in paragraph (D) of rule 3745-205-57, paragraphs (C) and (D)(6) of rule 3745-205-52, and paragraph (G) of rule 3745-205-53 of the Administrative Code, respectively, and the control device does not exceed or operate outside of the design specifications as described in paragraph (E) of rule 3745-205-64 of the Administrative Code for more than twenty-four hours, a report to the director is not required.
